Before filling out a registration form, check you are in our catchment area.

To register with the surgery:

• fill out an online registration form
• fill out a new patient questionnaire
• Paper registration forms are also available from reception

You will be required to provide us with proof of identity. This can be any of the forms listed on the registation form which you have completed.

Patients on 3 or more prescribed medications are asked to make a routine new patient medication review appointment.  Registration is with the practice, rather than a specific doctor.

We will only be able to discuss your confidential medical information with someone else if you have given us your written permission.
